General Requirements
Post graduate Degree in Pharmacy
Minimum 2 years’ experience in an acute care hospital
Should have valid registration in their home country
Accuracy and critical thinking
Effective communication and interpersonal skills
Proven ability to work effectively in a team environment and independently as required
Job Specification
Promote rational medicine therapy that is safe and appropriate
Monitor drug therapy comprehensively to optimize medicine therapy
Maintain drug formulary across the hospital
Perform medical audit of the medication management of the hospital that includes ADR, antibiotic policy adherence, errors related to prescription, dispensing, transcription and administration
Provide a comprehensive clinical pharmacy service to medical wards & critical care unit
Identification of prescription errors which includes route, frequency and dosage calculation
Daily reports of errors in medication management (which includes prescription, transcription and dispensing errors)
