Job Description
General Requirements
Post graduate Degree in Pharmacy as Doctor of Pharmacy (Pharm D)
Minimum 2 years’ experience in an acute care hospital as a Clinical Pharmacist
Should have valid registration in their home country
Accuracy and critical thinking
Effective communication and interpersonal skills
Proven ability to work effectively in a team environment and independently as required
Job Specification
Promote rational medicine therapy that is safe and appropriate
Monitor drug therapy comprehensively to optimize medicine therapy
Maintain drug formulary across the Hospital in terms of addition, deletion, slow/non-moving items.
Perform medical audit of the medication management of the hospital that includes ADR, Antibiotic policy adherence, errors related to prescription, dispensing, transcription and administration
Provide a comprehensive clinical pharmacy service to the medical wards and critical care unit at Hospital
Identification of the Prescription errors which includes route, frequency and dosage calculation
Daily reports of the errors in Medication management (which includes prescription, transcription and dispensing errors)
Patient counselling
