Job Description

General Requirements

  • Post graduate Degree in Pharmacy
  • Minimum 2 years’ experience in an acute care hospital
  • Should have valid registration in their home country
  • Accuracy and critical thinking
  • Effective communication and interpersonal skills
  • Proven ability to work effectively in a team environment and independently as required

 

Job Specification

  • Promote rational medicine therapy that is safe and appropriate
  • Monitor drug therapy comprehensively to optimize medicine therapy
  • Maintain drug formulary across the Hospital in terms of addition, deletion, slow/non-moving items.
  • Perform medical audit of the medication management of the hospital that includes ADR, Antibiotic policy adherence, errors related to prescription, dispensing, transcription and administration
  • Provide a comprehensive clinical pharmacy service to the medical wards and critical care unit at Hospital
  • Identification of the Prescription errors which includes route, frequency and dosage calculation
  • Daily reports of the errors in Medication management (which includes prescription, transcription and dispensing errors)
  • Patient counselling
